Beta-adrenerge rezeptor-Blockern (auch bekannt als beta-Blocker) sollten generell nicht verwendet werden bei Patienten mit manifester Herzinsuffizienz (CHF). Sympathische stimulation kann wichtig sein bei der Aufrechterhaltung der hämodynamischen Funktion bei diesen Patienten, also beta-blockade verschlimmern kann der Herzinsuffizienz. Jedoch wird die Therapie mit beta-Blockern kann vorteilhaft sein und verabreicht werden kann vorsichtig in einige CHF-Patienten, vorausgesetzt, Sie sind gut ausgeglichen und empfangen von digitalis, Diuretikum, einem ACE-inhibitor, und/oder Nitrate. Carvedilol insbesondere indiziert für die Anwendung mit diesen Wirkstoffen in der Behandlung der leichten bis schweren Herzinsuffizienz von ischämischen oder kardiomyopathische Herkunft. Es gibt auch zunehmende Hinweise, dass die Zugabe von beta-blocker zur Standardtherapie verbessern können Morbidität und Mortalität bei Patienten mit Fortgeschrittener Herzinsuffizienz, obwohl es unsicher ist, ob die Wirksamkeit variiert stark mit den verschiedenen Agenten.
