Konsument:

Benzodiazepine werden von der Leber metabolisiert und die Metaboliten werden im Urin ausgeschieden. Chlordiazepoxide, clorazepate, diazepam, flurazepam und quazepam Unterziehen, oxidative N-dealkylierung zu aktiven Metaboliten sind deutlich länger, als die Stammverbindung. Diese Metaboliten dann eine weitere biotransformation zu pharmakologisch inaktiven Produkte vor der Ausscheidung durch die Niere. Therapie mit Benzodiazepinen verabreicht werden soll, vorsichtig an niedrigere anfängliche Dosierung bei Patienten mit eingeschränkter Nierenfunktion und/oder Leberfunktion. Agenten, die konvertiert werden, auf schwach aktive, kurzfristig handeln, oder die inaktiven Metaboliten werden vorzugsweise in eingeschränkter Leberfunktion. Lorazepam, oxazepam und temazepam sind konjugiert zu inaktiven Metaboliten, während alprazolam, estazolam und triazolam Unterziehen Hydroxylierung zu schwach aktiven oder inaktiven Metaboliten.

Trizyklische und tetrazyklische Antidepressiva (Tzas) sind bekannt zu Unterziehen Stoffwechsel in der Leber. Einige der Metaboliten, wie z.B. Imipramin, Clomipramin und Desipramin, kann pharmakologisch aktiv. Viele Metaboliten werden auch von den Nieren ausgeschieden. Es gibt sehr begrenzte Daten über die Verwendung von TCAs in Patienten mit Nieren und/oder Leber-Krankheit. Therapie mit TCAs sollte verabreicht werden vorsichtig in Patienten mit erheblich eingeschränkter Nieren-oder Leberfunktion. Eine Dosisanpassung kann notwendig sein.
