Konsument:

Calcium-Kanal-Blocker (CCBs) sind umfassend durch die Leber metabolisiert. Die Hälfte lebt von CCBs kann verlängert werden beträchtlich bei Patienten mit schwerer Leberinsuffizienz, mit dem Potenzial für signifikante Medikament Akkumulation. Darüber hinaus ist die Verwendung von einige CCBs wurde im Zusammenhang mit Erhöhungen der serum-Transaminasen, sowohl mit als auch ohne gleichzeitige Erhöhungen der alkalischen phosphatase und des bilirubins. Während diese Effekte werden können, transient und reversibel, mehrere Patienten entwickelt haben, Cholestase oder hepatozelluläre Verletzung, die nachgewiesen wurde durch rechallenge. Therapie mit CCBs sollten verabreicht werden, mit Vorsicht und Häufig in reduzierten Dosierung bei Patienten mit deutlich eingeschränkter Leberfunktion. Eine regelmäßige überwachung der Leberfunktion und für exzessive pharmakologische Effekte (z.B. abnorme Verlängerung des PR-Intervall) wird empfohlen, und die Dosierung gegebenenfalls angepasst werden.

Der Einsatz von HMG-CoA-Reduktase-Inhibitoren ist kontraindiziert bei Patienten mit aktiver Lebererkrankung oder ungeklärte, anhaltende Erhöhungen der serum-Transaminasen. HMG-CoA-Reduktase-Inhibitoren sind umfassend durch die Leber metabolisiert. Verringerte Verstoffwechselung von Medikamenten kann dazu führen zu einer Akkumulation und erhöht das Risiko von Toxizität, einschließlich biochemische Anomalien der Leberfunktion und, selten, die Gelbsucht, hepatitis, Zirrhose, fettige Veränderung der Leber, und zu einer fulminanten hepatischen Nekrose. Therapie mit HMG-CoA-Reduktase-Inhibitoren verabreicht werden vorsichtig in Patienten mit einer Geschichte von Erkrankungen der Leber und/oder starkem Alkoholkonsum. Eine niedrigere anfängliche Dosierung, die geeignet sein könnten, und die klinische überwachung der Leber-transaminase-levels nach den individuellen Produkt-Paket-Kennzeichnung wird empfohlen. Patienten, die Entwicklung erhöhten ALT-oder AST-Spiegel während der Therapie überwacht werden soll, bis die Anomalien auflösen. Wenn ein Anstieg über 3-fache der oberen Grenze des Normalwertes fortdauert, überlegung sollte gegeben werden, um eine Verringerung der Dosierung oder der Entzug Therapie.
