Antihistaminika oft haben anticholinerge Aktivität auf, die ältere Patienten sind besonders empfindlich. Die Therapie mit Antihistaminika gegeben werden sollte vorsichtig, wenn überhaupt, bei Patienten mit vorbestehenden Erkrankungen, die wahrscheinlich noch verschärft werden durch die anticholinerge Aktivität, wie Harnverhalt oder Verstopfung; Engwinkelglaukom, unbehandelte intraokulare Hypertonie, oder unkontrollierte primäre Offenwinkelglaukom; und Magen-Darm-obstruktive Erkrankungen. Konventionelle, first-generation antihistamine wie die Ethanolamine (bromodiphenhydramine, carbinoxamine, Clemastin, dimenhydrinate, Diphenhydramin, doxylamine, phenyltoloxamine) dazu neigen, weisen erhebliche anticholinerge Effekte. Im Gegensatz dazu, ist die neuere, relativ nonsedating Antihistaminika (z.B. Cetirizin, Fexofenadin, Loratadin) haben angeblich niedrige minimale anticholinerge Aktivität auf, die normalerweise empfohlenen Dosierungen und können geeignete alternativen.
