Die zentralen dopaminergen blockierenden Effekte der Phenothiazine, kann herbeiführen oder verschlimmern kann, einen potenziell tödlichen Symptomenkomplex, bekannt als Malignen neuroleptischen Syndrom (NMS). NMS beobachtet wird, Häufig auf, wenn hohe Potenz neuroleptische Wirkstoffe wie haloperidol oder fluphenazine intramuskulär verabreicht werden, sondern kann auftreten, mit jeder agent besitzen Neuroleptika gegeben, für irgendeine Länge von Zeit. Klinische Manifestationen der NMS gehören hyperpyrexia, Muskel-Steifigkeit, veränderten mentalen status und Autonomer Instabilität (unregelmäßiger Puls oder Blutdruck, Tachykardie, Schwitzen und Herzrhythmusstörungen). Phenothiazin-Therapie sollte nicht begonnen werden bei Patienten mit aktiver NMS und sollte sofort eingestellt, wenn der aktuell verabreicht wird, in solchen Patienten. Bei Patienten mit einer Geschichte von NMS, Einführung oder Wiedereinführung von Phenothiazin sollte sorgfältig geprüft werden, da die NMS kann wiederkehren.
