Bestimmte Bedingungen wurden als Ursachen der reduzierten Theophyllin-clearance. Sie umfassen Alters (Neugeborene und Säuglinge < 1 Jahr sowie bei älteren Patienten > 60 Jahre) und die darauf folgende gleichzeitige Erkrankungen: akutes Lungenödem; dekompensierte Herzinsuffizienz, cor pulmonale, Fieber (>= 102 ° C für 24 Stunden oder mehr, oder weniger Temperaturwerte für längere Zeit); influenza; unbehandelte oder unkontrollierte Hypothyreose, Leber-Krankheit, Leberzirrhose oder akute hepatitis; eingeschränkter Nierenfunktion bei Säuglingen < 3 Monate alt; sepsis mit multi-organ-Versagen; und Schock. Therapie mit theophyllinen, sollten verabreicht werden vorsichtig in Patienten mit einer oder mehr dieser Risikofaktoren, und die Dosis sollte entsprechend reduziert, um zu verhindern, dass die Toxizität. Eine intensivere überwachung des serum-Theophyllin-Konzentrationen erforderlich sein. Die Toxizität ist am wahrscheinlichsten auftreten, wenn die Werte über 20 µg/mL. Schweren Fällen, manchmal ohne Vorwarnung, haben dazu geführt, Herzrhythmusstörungen, hartnäckigen Anfälle und Tod.
