Längere Verwendung von Kortikosteroiden kann die Ursache hinteren subcapsular Katarakt und erhöhter intraokularer Druck, von denen die letztere kann dazu führen Glaukom und/oder Schäden an den Sehnerven. Die Therapie mit topischen Kortikosteroiden selten erzeugt diese Effekte aber sollte verabreicht werden dennoch vorsichtig in Patienten mit einer Geschichte von grauem Star, Glaukom oder erhöhtem Augeninnendruck, insbesondere als potente Substanzen (z.B. augmented Betamethason, clobetasol, diflorasone, und halobetasol) sind in der periorbitale Bereich. Topische Kortikosteroide können systemisch absorbiert, je nach Fahrzeug und Konzentration der Vorbereitung, die Größe der Einsatzbereich, die Dauer der Verabreichung, und ob oder nicht okklusiven Verbänden verwendet werden. Äquivalenter Dosen, kleine Kinder sind in der Regel die größte Gefahr aufgrund Ihrer größeren Hautoberfläche zu Körpermasse-Verhältnisse.
