Der Einsatz von hydralazin wurde im Zusammenhang mit der Entwicklung von lupus erythematodes und lupus-ähnliche Syndrome, sowie Verschlimmerung der Krankheit. Hydralazin-Therapie sollte zurückgenommen werden, in der Patienten mit Verschlechterung der vorbestehenden lupus. Überwachung, komplette Blut zählt, und antinukleäre Antikörper-Titer vor und während der langwierigen Therapie empfohlen wird.
